SJDSS BCN is a social service provider in the homeless sector belonging to the Hospitaller Order of Saint John of God (HOSJG). The organisation’s mission is to provide comprehensive and person-centred care and support to people at risk or in situations of poverty and social exclusion, to promote opportunities for personal and social development so that they can pursue and achieve their life plans.
Role: As project coordinators, SJDSS is responsible for ensuring that the project is carried out effectively and according to the objectives set out. They are also responsible for supporting 25 homeless service providers with funding, training, and resources to integrate the COMHOM’s digital tools and the M&E Framework.
FSJD supports and manages the research and innovation activities of fourteen HOSJG entities in Spain to advance the healthcare and wellness of people.
Role: As an entity affiliated to the coordinator, it is responsible for assisting with post-award management and coordinating the sub-granting call.
VOH CoLAB is an R&D organisation whose mission is to help people and institutions measure value in health and transfer knowledge from academia to society. Driven by transdisciplinary approaches and digital transformation, VOH CoLAB works to develop collaborative contexts between healthcare teams and technology innovators to design and validate high-value innovations.
Role: VOH CoLAB is responsible for the social innovation and co-creation approach and will be explicitly involved in the establishment and monitoring of the COMHOM Living Lab, stakeholder engagement and co-creation activities.
SJDVAL is a social service provider in the homeless sector, belonging to the HOSJG. SJDVAL is dedicated to the care of homeless people through social programmes that address their specific needs. It conducts applied research to improve the care and quality of life of the people it serves.
Role: SJDVAL is responsible for creating a standardised M&E Framework, based on academic literature and the insights gathered from the Living Labs following a bottom-up approach.
SOCIAL IT is an SME dedicated to deploying and introducing innovative ICT solutions for managing social and healthcare services, focusing on the supply of services to the person.
Role: SOCIAL IT is responsible for developing the COMHOM digital solutions incorporating the insights gathered from the Living Labs.
PRAKSIS is a homeless service provider defined as an independent civil society organisation that aims to eradicate
the social and economic exclusion of vulnerable social groups and to defend their personal and social rights. Through the planning and implementation of projects of a developmental, humanitarian, and medical nature, PRAKSIS focuses on three main dimensions: prevention, intervention, and advocacy.
Role: PRAKSIS will be involved in all parts of the project, contributing its expertise to ensure that the design and implementation of the digital solutions is adapted to the needs of the sector.
ESN is a European network of public authorities, comprising 184 members from 34 countries which provide, finance, and manage local social and care services. ESN members work with different population groups, including children in need, adults with disabilities, homeless people and older people, to support the development of effective social services and care practices.
Role: ESN will lead on Communication & Dissemination for the project, policy recommendations, and exploitation efforts to maximise COMHOM’s impact. It will also be involved in the Living Lab activities through its extensive stakeholder network.
CHINO is an SME dedicated to helping organisations to solve security and privacy compliance challenges when dealing with sensitive special categories of data. CHINO offers a wide range of services, from legal (ethics, privacy, compliance) and technical security consulting to the provision of data security technology.
Role: CHINO is responsible for ethics, privacy and security to ensure that the project complies with EU regulations.
SCI is a network of independent communities responding to local needs in homelessness prevention, emergency accommodation, health and well-being and supporting people to exit homelessness. SCI is a leading provider of services to people who are homeless or at risk of homelessness and a leader in advocacy and campaigning for the rights of homeless people, working to raise awareness of homelessness throughout Ireland.
Role: SCI plays a role in the development of the M&E framework based on the relevant experience with the implementation of the Pathway Accommodation and Support System (PASS), an online shared system utilized by every homeless service provider and all local authorities in Ireland.
METRO is the largest university of applied sciences in Finland, with four education specialities: business, culture, health and social services, and technology. Its collaboration with key stakeholders in the social care and health care sectors generates new product innovations, operational models, methods, environments, tools and services, and knowledge.
Role: METRO is responsible for designing and implementing the capacity building programme to equip social care professionals with the necessary skills to implement and use the COMHOM digital tools and the M&E Framework within their organisations.
